Lipase catalyses the hydrolysis of triglycerides — the fat stored within adipocytes — into fatty acids and glycerol. These byproducts are then naturally processed and eliminated by the body’s lymphatic system, resulting in a measurable reduction in localised fat volume in the treated area.

Lipase works by enzymatic hydrolysis of ester bonds in triglycerides, producing free fatty acids and monoglycerides cleared via the lymphatic system.

Every enzyme session at 22 Harley Street follows a precise, medically supervised pathway — from initial assessment through to aftercare and review.

The five steps

  • Consultation & anatomy assessment — goals mapped, protocol designed
  • Preparation — area cleansed, topical anaesthetic applied if needed
  • Enzyme injection — lipase into the fat layer, collagenase into fibrous tissue
  • Post-treatment care — gentle massage, aftercare guidance provided
  • Course of 3–6 sessions, spaced 4–6 weeks apart

  • Days 1–5: localised swelling and tenderness — normal and expected

Results develop over time. Enzyme therapy works through biological mechanisms that take weeks to fully express. Most patients see the clearest improvements 4–8 weeks after each session, with the full course result established at around 3–6 months.

Lipase and collagenase are naturally occurring enzymes already present in the human body. When used in controlled therapeutic doses by qualified medical practitioners, enzyme therapy has a well-established safety profile. A thorough medical assessment is always conducted before treatment.
Most patients describe the injections as mild and well-tolerated. A topical anaesthetic cream can be applied beforehand for additional comfort. Some tenderness and swelling in the days following treatment is normal and indicates the enzymes are working.
Most patients benefit from a course of 3–6 sessions, spaced 4–6 weeks apart. The exact number depends on the area, the extent of the concern and individual response — this will be discussed and planned during your consultation.
Downtime is minimal. Some swelling, redness and tenderness in the treated area is expected for a few days following each session. Most patients return to normal activities the same day or the day after. Strenuous exercise is best avoided for 48 hours post-treatment.
Yes — in many cases, combining both enzymes in the same treatment zone is the most effective approach, addressing fat volume and skin texture simultaneously. The protocol is designed individually at consultation and may use one or both enzymes depending on your specific concerns and anatomy.
Fat cells dissolved by lipase do not regenerate. However, remaining fat cells can enlarge with significant weight gain, so maintaining a stable weight helps preserve results. Collagen remodelling results are long-lasting but the skin continues to age naturally over time. Maintenance sessions can be considered periodically.
Unlike energy-based treatments such as cryolipolysis or HIFU, enzyme therapy works through a purely biological mechanism — using the body’s own pathways. It is particularly well-suited to areas requiring precision and to patients who also want skin texture improvement alongside fat reduction.
